The Well-Being Assessment is a patient assessment designed to capture the patient's goal, define a care plan, ensure the plans are confirmed by care team and patient.

  • Goals - Quality of Life (personalized definition of Personal Well-Being), Life Goals, Care Goals
  • Care Plan - Interventions, Who does what?, How it achieves goals, how it is measured, Who is part of the Care Team
  • Synchrony - How it fits into patients life and everyone's life, ensure alignment and in sync
