Health Status
From Patient Determinants
In order to measure health outcomes, we need to measure health status before and after interventions (Medical &
Non-Medical). The following are components of health status
- Symptoms (Patient) - This is the patient's view of their physical and mental symptoms. They include pain or discomfort, visual symptoms, cold or flu-like, physical abnormalities, mental health and other.
- Conditions (Medical) - These are disease and conditions (i.e., diabetes, hypertension) as well as biomarkers (i.e., blood pressure, weight, blood glucose) as documented by a medical professional in the patients Electronic Medical Record.
- Risk (Medical/Patient) - These are Medical Risks (i.e., for disease, disease progression, acute event, mortality) and Determinant Risk (i.e., patient behavior, medication adherence, nutrition, transportation, housing).
- Function (Patient) - This is the patient's assessment of their function (i.e., walking, going to work, doing what they love).
- Well-Being (Patient) - PHQ, Purpose, Love & Connection, Competence
