These are performance measures to help determine the impact on the patient's health status:

1. Initial Health Status - This is the measure of the risk-adjusted health status of the person prior to a determined timeframe, a new mission (Care Goals), a new strategy (Care Plan) or a new solution (intervention). The will be the baseline used to measure the health outcome.

3. Health Strategy - Objectives (Care Plan)

  • List of Strategy Objectives

4. Health Strategy - Solutions (Interventions) - The solutions are a subset of the objectives. They require their own specific measures to ensure that are effective.

5. Health Strategy - Measures

  • Objective Measures - These are measures that will demonstrate progress toward achieving the health mission. This is the list of measures (i.e., pain levels, function levels, blood sugar levels) that will be used to measure progress of the startegy objectives toward achieving the health mission.
    • Objective Measurement Timeframes - These are the times when progress will be measures
    • Objective Measurement Baseline - This is the initial measures that will be used to measure the health outcome of the strategy objectives.
  • Solution Measures - These are measures that will demonstrate progress toward achieving the health mission. This is the list of measures (i.e., pain levels, function levels, blood sugar levels) that will be used to measure progress of the startegy objectives toward achieving the health mission.
  • Solution Measurement Timeframes - These are the times when progress will be measures
  • Solution Measurement Baseline - This is the initial measures that will be used to measure the health outcome of the strategy objectives.
