Patient assessments are designed to capture patient information for a particular condition (such as COPD Assessment) or service (such as an Emergency Room, Hospital or Physician Office Visit). The Centers for Disease Control and Prevention (CDC) developed a Health Risk Assessment (HRA) designed for the Medicare Annual Wellness Visits.
For Managing Health, the following assessments (or methods for capturing patient data will be developed:
- Managing Health Assessment is designed to understand the patient by documenting their current determinants and interventions.
- Well-Being Assessment is designed to capture the patient's goals (quality of life, care goals), care plan (goals, determinants, interventions, care team) and ensure alignment (patient, caregiver, physician, care team).
- Well-Being Status is designed to document daily status (how are you feeling, symptoms), actions (what happened?, what didn't happen?) and changes (symptoms, barriers, goals, determinants, interventions, plans).
- Functional Assessments - understanding of the Katz Index of Independence in Activities of Daily Living (ADL) and Lawton - Brody Instrumental Activities of Daily Living Scale (IADL).

- CDC Health Risk Assessments is a comprehensive patient assessment completed as part of an annual wellness visit.
- CDC Health-Related Quality of Life (HRQOL) is an individual's or a group's perceived physical and mental health over time. It promotes Measuring Healthy Days for Population Assessment of Health-Related Quality of Life
Other Assessments
- Depression Anxiety Stress Scales (DASS) - is made up of 42 self-report items to be completed over five to ten minutes, each reflecting a negative emotional symptom.
- Short Form Health Survey (SF-36) - is a set 36 items of generic, coherent, and easily administered patient reported quality-of-life measures utilized by managed care organizations and by Medicare for routine monitoring and assessment of care outcomes in adult patients.
- EQ-5D - is a standardized instrument developed by the EuroQol Group as a measure of that can be used in a wide range of health conditions and treatments comprising five dimensions: mobility, self-care, usual activities, pain/discomfort and anxiety/depression.
