Support Services are support focused services or capabilities designed to meet specific support needs of patients based on the support services' criteria. These services may or not be integrated with the patient's care plan or with an E Intervention Program. The criteria includes risk scores, episode, health conditions, Non-Medical Intervention Programs enrollment and determinants. These are support services typically provided by community and social services to address non-medical determinants. These support services are arranged and coordinated by providers to their attributed patient populations to improve health outcomes.
