In order to measure health outcomes, we need to measure health status before and after interventions (Medical &
Non-Medical) and biological age and health status on each birthday. The following are components of health status:
- Symptoms (Patient) - Patient's assessment of their physical and mental symptoms. They include pain or discomfort, visual symptoms, cold or flu-like, physical abnormalities, mental health and other.
- Conditions (Medical) - These are disease and conditions (i.e., diabetes, hypertension) as well as biomarkers (i.e., blood pressure, weight, blood glucose) as documented by a medical professional in the patients Electronic Medical Record.
- Functional Status (Patient) - Patient's assessment of their function (i.e., walking, going to work, doing what they love).
- Clinical Risk (Medical) - These are Medical Risks (i.e., for disease, disease progression, acute event, mortality)
- Determinant Risk (Patient) - Patients assessment and monitoring of patient behavior based on health determinants (i.e., fiancial, transportation, housing).
