Support Services are support focused services or capabilities designed to meet specific support needs of patients based on the support services' criteria. These non-medical interventions services may or not be integrated with the patient's care plan or with a Care Management Program. The criteria includes risk scores, episode, health conditions, Care Management Programs enrollment and health determinants. These are support services typically provided by community and social services to address non-medical determinants.
